ARCB SOAP Note Form and SamplesA requirement for the ARCB Certification application process is to successfully complete a digital SOAP Note supplied by the ARCB A Client Background and the front side of the SOAP Note will be provided. Applicants will then be required to successfully complete the back side of the SOAP Note and submit as part of the initial application.
